Yes -unbelievable - some people should not have children !

I suppose this is just one of the examples the authorities let slip through the net - that is, until a death occurs.
Then they decide to intervene.

It's very similar to the the Highways Department of the government, until their are a few deaths after an accident on a specific road, they won't investigate why it keeps happening and change either the road layout/or priorities/or speed restrictions etc. They appear to wait for casualties to rise.

Unfortunately you're not even safe if you go into foster care here in Ireland, as there don't appear to be any checks on their suitability to be foster care providers...as for being under the care of the HSE, I don't think we'll even go there!
